What types of insurance plans do you accept?

What should I bring with me to my appointment?

We request that you bring your insurance card, photo identification, any relevant medical records from other health facilities, and a method of payment for your portion of financial responsibility.

Does the office accept credit cards for payment?

Yes. We accept Visa, Mastercard, American Express, and Discover, in addition to cash, checks, money orders, or CareCredit. Checks should be made payable to Shariar Cohen, MD. Returned checks will be assessed a $25 fee.

What if I will be late for my appointment?

If you will be late, please call our office as soon as you know. We will do our best to accommodate you but we may have to reschedule you to another convenient time.

Do I have to pay at the time of my appointment?

Yes. Any deductibles, copayments, or co-insurance are due at the time of service. The specific amount of your financial responsibility is determined by your health plan as either a fixed dollar amount or a fixed percentage of the contract allowable amount. Please refer to your health plan for more specific information on financial responsibility.
